Enrollment Packet

This packet contains enrollment paperwork to be given to all participating learners and families by their Resource Consultant. Detailed information regarding these forms can also be found in the Vision HCP Handbook.

Enrollment paperwork must be completed for each Learner. Learners who were enrolled last year need to complete all enrollment paperwork for the 2006-2007 school year. Vision HCP contracts are written for only one year and forms must occasionally be updated and changed.

This packet only contains forms for the initial enrollment period.
