 HOME AND COMMUNITY PROGRAM

Contract of Agreements 2006-2007

 

 

Purpose: To develop and maintain a working relationship between the Learner and the Surface Creek

    Vision Home and Community Program.

 

 

I.  The Surface Creek Vision HCP agrees to:

 

A.        Conduct relationships based upon integrity and respect for participantsŐ educational beliefs and  methods.  Recognize that it is the primary responsibility of the parent to choose the proper education and training for children under their care.

B.  Find the highest quality Resource Consultants and fully support them in their role as an advocate for the Learner and his or her family.

C.  Work with the Learner to find a Resource Consultant to his or her liking.

D.  Support the LearnerŐs option to receive a public school diploma should he or she complete necessary requirements.

E.  Support each full-time Learner with access to up to $2150 in funds for educational purposes if the Learner is enrolled by October 1.  After October 1, each full-time Learner will be supported with access of up to $650.

F.  Continue to support the home-school law and legitimate schooling choice within and outside the public school system and Vision.  The Vision HCP recognizes that some of the requirements for participation in Vision are above and beyond the home-school law (i.e. taking the CSAPS), and we will work to minimize such requirements in the future.

 

 

II.  Participants in the program agree to:

 

A.  Maintain a cooperative, working relationship with a Resource Consultant. This includes developing and abiding by a working agreement.  The funding for this program is contingent upon meeting the criteria outlined in this agreement and in the agreement with a Resource Consultant.

B.  Maintain and demonstrate integrity both personally and for the program in all required paperwork and relationships.

C.  Document actual learning hours and meet minimum requirements:

q      360 learning hours per semester for full-time enrollment with 90 hours per semester with someone outside the LearnerŐs immediate family.

q      Minimum of 90 learning hours per semester for part-time enrollment with 45 hours per semester with someone outside the LearnerŐs immediate family.

q      Complete all required paperwork such as: Application Form, Enrollment, Release of Information Waiver, Child Protection Screening Form, Immunization Form, this Vision HCP Contract, Learner/Parent/Resource Consultant Agreement, Basic Skills Assessment Summary, Learning Plan, Learner Schedule, Learner Crossover Agreement (if necessary), provide copies of receipts and completed funding requests, and Attendance Records with necessary documentation. 

q      Complete Learning Updates two times per year and Learning Plan Reflections two times per year.  Complete a transcript at the end of the year.

q      Work with the Resource Consultant to keep track of LearnerŐs basic skill levels and take the

State-mandated CSAP and ACT tests at the LearnerŐs appropriate level once per year.

 

III.  Learner, Parents and Resource Consultant agree to:

Develop a plan for someone to meet with the Learner at least once per week to support him or her in his or her learning program.  The RC will participate weekly, semi-weekly, or monthly depending on a mutually determined decision.

IV.  Funding

In order for the Vision HCP to operate with integrity, it is important that all parties are in consensus about the funding process.  It is imperitive that:

¤       the Learner, parents, and Resource Consultant be in 100% agreement about what items should be funded;

¤       the need for these items is specified in the Learning Plan, which is created by consensus of the aforementioned parties;

¤       all parties can stand fully behind the decision to purchase these items with state funds, and

¤       if anyone at anytime is not comfortable funding an item, they are given time to seek support about their questions and uncertaintanty.  At no time should anyone feel pressure to authorize funding of any item.

¤       Funds are intended to be used for educational purposes.  Items bought using Vision HCP money ultimately belong to the program and are not to be sold for personal profit.

 

V.  Lack of Compliance

If any party (Vision, Parent or Learner) fails to meet the requirements of this program or rapport is broken with the Resource Consultant, then:
